Overview

UCPLA is currently hiring a Registered Nurse Case Manager. The RN Case Manager’s primary responsibility will be case management of residential clients with developmental disabilities in Intermediate Care Facilities/ Developmentally Disabled- Nursing. The RN Case Manager will function in the role of the facility RN intermittently, and may at times assist the primary RN with the following: assessing client medical condition and needs, developing and implementing nursing care plans, and maintaining medical records, as well as intermittently providing direct nursing care to clients. The ideal candidate will have at least 2 years’ experience providing direct RN care and complex case management to people in a long-term care and/or acute care facilities.

  • Our facilities are Intermediate Care Facilities (not SNFs) for the Developmentally Disabled, with both Habilitative (ICF/DD-H) and Nursing (ICF/DD-N) homes    
  • Our ratios:
    • Caregiver to client ratio does not exceed 1:5 and is sometimes as high as 1:1
    • Nursing caseload is 15-30
    • Our home sizes are small (6-bed and 15-bed)
  • At our long-term care facilities, you have opportunities to build relationships with people with developmental disabilities and a team of staff committed to service
  • Complex case management will keep you engaged and learning
  • High-acuity care is limited or prohibited by facility regulations
  • Our Intermediate Care Facilities provide wound care per licensing requirements ranging from simple wounds only to a maximum of stage II pressure ulcers
  • Hospice care is very limited; when provided, it is done collaboratively with professional hospice care providers who come to our homes

 

 

 

Responsibilities

  • Provides direct RN assessment and care management of each resident for a  minimum for a 1-1.5 hours per week
  • Provides complex case management to residents in an ICF/DD-N facility working in a collaborative manner with RN colleagues, nursing staff, Administrator, Directors, and management team
  • Responsible, in collaboration with the primary RN team, for all components of the residents’ individual health care needs
  • Promotes quality care and excellence in nursing following UCPLA policies and National Standards of Nursing Care
  • Responsible for managing the LVN shift schedule in coordination with the Administrator and RN team
  • Participates in the development and presentation of Individual Service Plans
  • Audits all medication documentation and orders for safety and compliance as required by State and Federal regulations
  • Collaborates with the primary RN team in reviewing medical record audits
  • Collaborates with the primary RN team in reviewing documentation of medication dispensing and drug regimen for each resident as required by regulations
  • Serves as a member of the Interdisciplinary Team at Individual Service Conferences
  • Occasionally assists primary RN in completing quarterly nursing reports, including assessment and documentation
  • When needed, effectively supervises nursing care being given by subordinates
  • Interacts effectively with Direct Care Staff and other facility staff to keep them informed of clients’ medical needs, precautions to be taken, and observations to report
  • Provides in-service training in the areas of client health needs, emergency procedures, and infection control as determined by the Administrator

Qualifications

  • Valid California RN License
  • One (1) to two (2) years of full-time clinical experience as an RN
  • Excellent oral and written skills
  • Strong critical thinking skills
  • Ability to provide complex case management for medically involved and fragile clients
  • Experience working with adults with disabilities and/or developmental disabilities is preferred
  • Ability to lift fifty (50) pounds
  • Ability to successfully pass all post-offer pre-employment screening requirements, including criminal background check, drug screening, and physical exam, and to provide proof of eligibility to work in the United States
  • Able to run, push, pull, bend, squat, kneel, lift and transfer clients (with assistance/Hoyer Lift)
