UCPLA is currently hiring Floating Direct Support Professionals (FDSS) to work closely with individuals with disabilities in their private residence from 10:00 PM to 6:00 AM; Sunday to Monday: 2:00 PM to 10:00 PM; Thursday to Friday and 10:00 AM to 6:00 PM on Saturday. FDSS provide scheduled and intermittent services to a group of individuals in various apartment units in one location as scheduled and as needed. As a FDSS, you will encourage our clients to become as independent as they can be while promoting community engagement. You will be providing support to people served in their homes and in the community by providing training and assistance with personal care and housekeeping, communication, supervision, companionship, and behavior support. Under the direction of a Community Living Specialist and other highly qualified professionals, you will develop methods to maximize client independence and meet goals identified in their Individual Service Plan.

Hours are:

Sunday - Monday;  10:00 PM - 6:00 AM

Thursday - Friday; 2:00 PM to 10:00 PM

Saturday; 10:00 AM to 6:00 PM 

• Provides scheduled and intermittent services to a group of individuals in various apartment units in one location as scheduled and as needed
• Responds to calls for assistance from people served appropriately and within the required timeframe
• Assists people served in achieving personal support goals at home as identified in the Individual Service Plan (ISP) and the Individual Program Plan (IPP)
• Assists people served to maintain their personal appearance, personal hygiene, and health at the highest possible level by providing support in all areas of personal care
• Assists people served with household duties and to maintain their residence in a clean, orderly, and safe condition
• Assists people served to monitor and maintain their health
• Ensures the safety and well-being of people served at all times at home
• Communicates professionally, effectively, and promptly with others
• Assists in the preparation of incident reports
• Completes all required documentation, including general logs, financial logs, medication administration records, and progress notes/flow charts
• Accesses electronic communication, including but not limited to electronic records, online payroll processes, and online schedules

• High school diploma or equivalent required
• Sufficient reading, writing, comprehension, and communication skills in English to perform essential duties and responsibilities
• Availability to attend required in-service training
• Ability to maintain confidentiality
• Ability to communicate and work professionally and cooperatively with others
• Ability to successfully pass all post-offer pre-employment screening requirements, including criminal background check, drug screening, physical exam, and provide proof of eligibility to work in the United States
